1. Clinicians confirming eligibility for PANTHER must first complete the Eligibility Training Slides  
2. Clinicians confirming eligibility for PANTHER must receive a copy of the latest Master Protocol , Simvastatin ISA , Baricitinib ISA
3. By completing this training log the clinician agrees with the following ‘I confirm that I work at the hospital site listed by site number below within the start and stop dates of the delegation log’ 
4. The Clinician must then complete this form to confirm that the Eligibility Training has been received and understood. 
5. The Clinician must sign the site delegation log 
6. When confirming eligibility, the PANTHER Eligibility Form must be completed for each patient, signed by the clinician to confirm eligibility and filed in the medical records.
